The Endocrine Society's independent appraisal cuts to the heart of a tension that will affect millions of adults: does a more precise definition of obesity improve care, or inadvertently create barriers to it?

The Lancet framework proposes retiring BMI as the primary diagnostic criterion, replacing it with direct or surrogate measures of excess adiposity. It then stratifies patients into two distinct categories: "clinical obesity," defined by measurable organ or system dysfunction attributable to excess fat, and "preclinical obesity," where adiposity is elevated but objective dysfunction has not yet emerged. Validation analyses from large-scale biobanks — including All of Us and the UK Biobank — confirm that even preclinical classifications carry meaningfully elevated cardiometabolic risk, with clinical obesity conferring even higher risk. Central adiposity and fat distribution patterns outperform BMI alone in predicting metabolic harm.
